Referrals

Melbourne Project

As a partner organisation in the City of Yorks Single Access Point Project, Peasholme Charity is now asking for all referrals/applications for the Melbourne Project to be sent directly to Alice Walker at CYC. Alice, as coordinator for The Single Access Point will act as a “clearing house” for all Resettlement Service referrals and will ensure that they are passed on to the most appropriate service.

The new SAP application form , to be found on this website, replaces the Peasholme Shared Housing Application form that was previously in use.

In order that effective communication is maintained between all parties the following will apply:

  1. Upon a vacancy arising in the Melbourne Project, agencies will be notified that referrals will be accepted.
  2. Referring agencies should then contact the Project Manager to discuss a potential referral. It would be beneficial at this point to discuss the details of the referral in order to ensure the suitability of an individual to the project; this can be done in a client anonymous manner in order to maintain confidentiality.
  3. A referral pack can be downloaded from the links above or if requested can be posted. The referral should be returned to the Melbourne Project as soon as possible.
  4. The Project Manager will then contact the referring agency to arrange an interview/assessment for the applicant.
  5. Following the submission of an application, a referring agency may at any stage contact the Project Manager to discuss individual applications.
  6. If an individual has not made a successful application then the referring agency will be contacted and given specific reasons why.
  7. All information will be treated as highly confidential.
