Referrals Melbourne Project
The information below refers to the Melbourne Project only.
Melbourne Project
The Melbourne Project would expect that referring agencies would have assessed potential tenants prior to submitting applications to ensure that the applicant fulfils the referral criteria. Also, it is expected that the individual will have been consulted and advised prior to referral and that he/she believes this to be a positive move for them at this present time.
Applications
Download Application Form
The application form is in two sections, which are available in both Word and PDF formats.
Please return both parts by post to the address on the form.
Word Format
PDF Format
If your computer does not open the PDF documents automatically, then download Adobe PDF Reader here.
Please read all the information on this page before sending in a referral form to the Melbourne Project.
In order that the customers needs can be properly assessed, it is important that the application form is completed with as much detail as possible. Please include any physical or mental health issues, or other relevant support needs. In particular please include details of arson, violent behaviour or sex offences, this information will not in itself exclude the referral from being a positive application. Referrals made on behalf of individuals with multiple substance misuse issues will be considered subject to a Risk Assessment.
In order that effective communication is maintained between all parties the following will apply:
- Upon a vacancy arising in the Project, agencies will be notified that referrals will be accepted.
- Referring agencies should then contact the Project Manager to discuss a potential referral. It would be beneficial at this point to discuss the details of the referral in order to ensure the suitability of an individual to the project; this can be done in a client anonymous manner in order to maintain confidentiality.
- A referral pack can be downloaded from the links above or if requested can be posted. The referral should be returned to the Melbourne Project as soon as possible.
- The Project Manager will then contact the referring agency to arrange an interview/assessment for the applicant.
- Following the submission of an application, a referring agency may at any stage contact the Project Manager to discuss individual applications.
- If an individual has not made a successful application then the referring agency will be contacted and given specific reasons why.
- All information will be treated as highly confidential.
