The Peasholme Centre Referral Policy

This policy aims to provide a clear framework for the processing of referrals to the Peasholme Centre. For the purposes of this policy, the term `Peasholme Centre` will include both, the Peasholme Centre Charity and the City of York Council.

It is hoped that a referring agency will have assessed any service user being referred to the centre for services as being suitable and able and willing to engage with the resettlement programme. Referring agencies will use `The Peasholme Resettlement Centre Referral form` and `Referring Agencies Risk Assessment form. It will be expected that both these forms will be completed in full. The customer being referred will be expected to sign a consent form for the Peasholme Centre to conduct a PNC check, which may need to be completed as part of a risk assesment

It will be expected that referring agencies will have resolved issues pertaining to Local Connection, and will have completed `Local Connection assessment tool` form alongside the referring forms. Written verification of queried cases must also be available.

The following procedure will occur when a referral is received by the Peasholme Centre:

  1. The referring agency will receive confirmation when the referral is received by the Resettlement Centre staff.
  2. An assessment of the referral will occur within ten working days of the referral being received. During this time further information will be sought if required.
  3. If the referral seems suitable at this point, an interview will be set up with the individual and two members of staff at the Resettlement Centre.
  4. If we accept the referral the individual will be offered access to day centre activities, and will be assigned a keyworker.
  5. Any day service user will be expected to undergo a comprehensive assessment and work with their keyworker in developing their resettlement plan.
  6. If the referral is rejected the reffering agency will be notified in writing as to the reasons why, and an outline will be provided as to what will be necessary for the referral to be succesful.
  7. Any information received by the Peasholme Centre will be held in accordance with the Peasholme Centre's confidentiality policy, and will only be used as part of an individual's assessment.
