Referrals
The information below refers to the Shared Housing Scheme only. For other referrals please contact us.
Shared Housing Scheme
It is expected that the referring agency will have assessed the suitability of the person they wish to refer. All referring agencies are asked to complete a referral form outlining the extent of any resettlement work undertaken and what further support may be available to the individual concerned.
Applications
Download Application Form
The application form is in two sections, which are available in both Word and PDF formats.
Please return both parts by post to the address on the form.

PDF Format
If your computer does not open the PDF documents automatically, then download Adobe PDF Reader here.
Please complete and return the application form in the first instance. Housing workers will then arrange an interview time and date.
In order that the client’s support needs can be properly assessed it is important that the application form is completed with as much detail as possible, including any physical or mental health issues, or any other relevant support needs. In particular please include details of violent behavior, drugs and/or alcohol abuse, or sex offences. This information will not in itself exclude the client from making a successful application, but will enable staff to allocate rooms appropriately. However, due to the shared nature of the accommodation we will not accept referrals with a history of arson.
In order that effective communication is maintained between all parties the following will apply:
- A housing worker will contact each interviewee’s referring agency within seven days of receipt of the application to discuss it. Any further information required will be addressed and the client will be either offered an interview or their application will be turned down. Any client not placed on the waiting list will be given a written explanation addressed to the referrer.
- Two SHS staff will interview each client. The referrer may accompany clients.
- SHS staff normally contact the referrer within 24 hours of the interview but when several interviews have taken place within a few days it may take longer. SHS staff will inform the referrer whether or not the client has been accepted onto the waiting list.
- If there is a specific issue that needs to be addressed before a client could be offered a place in the scheme, their referring agency will be informed. No offer will be made until the issue has been addressed. There may be a requirement for the client to be re-interviewed.
- If a client is not suitable for the scheme their referring agency will be informed and given a written explanation of the specific reasons why this decision has been taken.
- All offers of accommodation will be made in writing.
