This statement explains what personal information we record about you, why we record it and how we use it.

The rules for how we manage your data are governed by the General Data Protection Regulation (GDPR) 2016.

 

Peasholme Charity holds your personal information; including your name, address and contact information.

This allows us to contact you in relation to the services we provide.  The organisation, also holds some personal sensitive information about you; this may include information about your physical or mental health, housing or financial information, and benefits information, where applicable information about your offending history. Your support worker will also keep records of your meetings, agreements and attendance records.

Why do we need this information?

 

Peasholme Charity aims to provide comprehensive support to you so we can help you achieve your goals, resolve any crisis situations you are currently facing and make positive changes to your life.  The information we collect helps us get to know you,

provide accurate advice and track your progress & achievements.

Peasholme Charity aims to be accountable to the people who fund its services; to do this, we collect information about our work.

 

 

How is my information stored?

 

Your support worker is responsible for collecting your information and storing it safely & securely in the organisation’s online data application.  This application is owned by Evide, a specialist ‘tech for good’ company, and is called the Impact Tracker.

Your support worker may keep handwritten notes from your meeting, these notes are inputted into the Impact Tracker and the written notes are destroyed.

 

 

Will you share my information with anyone else?

 

Peasholme Charity works in partnership with other agencies to deliver its services.

Your support worker is required to obtain your written consent in order to share your personal information with any third party.

There may be circumstances that information would be shared without your consent; this would only relate to keeping you and others safe.

Peasholme Charity would never share / sell or trade your personal information with a third party.

 

 

 

What happens to my data when I stop working with Peasholme Charity?

 

Your personal file will be closed and moved to an archive section on the Impact Tracker, it will be kept there for a minimum of 5 years.

Some data may be held for longer if contract policy, financial or legal regulations require it. It will never be kept for longer than is necessary.

 After 5yrs, and if you do not access another one of our services; then your file will be deleted.

Anonymised data will be kept and used to evidence the impact of the organisation’s work and for fundraising purposes.

 

 

Who can see my information?

 

Peasholme Charity employees can see your information and records; this includes the Charity Manager.

We use anonymised data and case studies for our funders so we can evidence the impact of the work we do.

In some circumstances, your data will be shared with funders.  You will be told about this in advance and will receive privacy

information from specific funders if this will affect you.

 

 

 

How do I go about seeing the information that you hold about me?

 

GDPR has been written to give you more control over your information and how companies like ours can use your data.

You can request to see your information at any point - just speak to your support worker.  When you ask to see your information,

we have one month to respond by providing you with a complete copy of your file.
